Your New Client Forms

We want your first visit to be 100% focused on you and your child, not on paperwork in the waiting room.

To make this happen, we've built a simple digital process.

Here’s How It Works:

1. .After your initial call with our intake coordinator, we will email you a secure link to our New Client Packet. → add the link to contact us here
2. Please click this link and complete all the forms online at least 48 hours before your scheduled evaluation.

Completing these forms in advance gives our therapist the important time they need to review your child's history and prepare for a great first session.

The packet you receive will include a few key documents:

• New Client Intake Form: This is where you'll share your child's health history, any developmental milestones, and your main concerns and goals.

• Consent for Treatment & Clinic Policies: This form explains our clinic policies (like cancellations and privacy) and gives us your official permission to begin therapy.

• Notice of Privacy Practices (HIPAA): This explains your rights regarding your child's protected health information.

• Release of Information (Optional): This optional form allows us to communicate with your child's doctor, school, or other specialists to help coordinate care.
